Jurisdiction Ohio Servsafe approval Yes (Print and Online exam with an approved instructor) Certification Fee Instructor Requirements The Ohio Department of Health (ODH) approves instructors and teaching bases, or locations. - Instructor must have a minimumof four years of training or teaching experience and be registered as a ServSafe Instructor. Code requirement Mandatory: Level One: For the person in charge per shift of a new food service or retail food establishment is licensed on or after March 1, 2010. ~ When establishment has been implicated in a food borne disease outbreak. ~ When licensor has documented a failure to maintain sanitary conditions per Ohio Revised Code section 3717.49 for a food service operation and Ohio Revised Code 3717.29 for retail food establishments. Voluntary: Level Two: Person in charge must be able to demonstrate knowldge and ensure employees are trained in food safety as it relates to their assigned duties - ServSafe Manager training needs to meet the requirements in rule 3701-21-25 of the Ohio Administrative Code. Hours of Instruction Minimum 15 training hours for Level Two Certification Certification Renewal ServSafe certificate is valid for 5 years. Currently the certification card from ODH is permanent in OH Miscellaneous In order to obtain the Level Two certifiction card from ODH scores must be directly provided to them. For the Level One certification all risk level I, II, III, and IV food operations that opened on or after March 1, 2010 must have at least one person in charge per shift.
